Live in or planning to purchase a home in Baltimore City? Complete the CDBG Income Verification Form in English or Spanish.

If this does not apply to you, simply continue to Step 5.

  • Valid ID — driver’s license or passport
  • Most recent 30 days of pay stubs or other proof of income

If self-employed, also provide:

  • 6 months of profit & loss statements
  • Most recent federal tax returns only
  • 1099s
  • 2 months of bank statements
